Role Overview
The Clinical Case Manager plays a vital role in delivering person-centered support coordination for clients and their carers. You will be responsible for helping clients retain their life skills, coordinating support services, and ensuring the delivery of high-quality care tailored to their unique needs.

Key Responsibilities
- Provide guidance and support to clients and their carers, facilitating informed decision-making.
- Assist clients in identifying goals, articulating support needs, and developing support plans.
- Monitor and review care plans to promote clients' well-being and social engagement.
- Foster relationships with clients, carers, and local service providers to enhance community connections.
- Conduct comprehensive assessments and maintain accurate records of client care.
- Participate in professional development activities to enhance knowledge and skills.
Candidate Profile
We are seeking a dedicated professional with strong interpersonal skills and a commitment to high standards of care. The ideal candidate will possess:
- Tertiary qualification in health, welfare, or related field, or equivalent experience.
- Current Registered Nurse (RN) status with AHPRA registration.
- Demonstrated experience in support coordination or team leadership.
- Knowledge of aged care standards, legislation, and person-centered practices.
- Excellent communication skills and the ability to work autonomously.
- Proficiency in digital applications and client information systems.
