Assistant in Nursing with Medication Competency – Support at Home (Aged Care)

Part-Time | Brisbane South - Mount Gravatt

We are seeking compassionate and committed Assistant Nurses to join our Support at Home Program, delivering care that is respectful, person-centred, and grounded in genuine human connection.

Working under the supervision and direction of our clinical care team, you will provide delegated, non-complex clinical care alongside personal and daily living support. Your work will make a meaningful difference to the lives of older people and their families every day.

The role is to Clinical Care Support to assist the support at home clients with as below: 

  • Provide delegated clinical care under the supervision and direction of the clinical care team, in line with scope of practice and organisational values.
  • Support safe medication assistance and monitoring in accordance with care plans and policies.
  • Assist with monitoring and documenting routine clinical observations under Registered Nurse direction, including:
    • Blood pressure
    • Temperature
    • Pulse and respirations
    • Blood glucose levels (BGLs)
    • Simple wound care
    • Application of compression garments
  • Monitor skin integrity and escalate concerns promptly, recognising the importance of early intervention.
  • Observe, document, and communicate changes in a client’s condition, ensuring timely escalation to the Registered Nurse for assessment.

Personal Care & Daily Living

  • Support clients with personal hygiene, mobility, transfers, toileting, dressing, and grooming, always preserving dignity and choice.
  • Assist with meal preparation and nutrition in line with individual preferences and dietary requirements.
  • Encourage independence and confidence, supporting clients to do as much for themselves as possible.

Person-Centred, Relationship-Based Care

  • Deliver care that reflects each client’s goals, life experiences, culture, and preferences.
  • Promote reablement, wellness, and independence consistent with the principles of the Support at Home Program.
  • Build trusting, respectful relationships with clients, families, carers, and the wider care team.

About You

You are kind, respectful, and committed to delivering care that truly puts people first. You understand the importance of working within scope, communicating clearly, and being a reliable and compassionate presence in someone’s home.

Essential Qualifications and Requirements:

  • Certificate IV in Health Services Assistance, Aged Care, or equivalent
  • Diploma of Enrolled Nursing (or working towards)
  • Current First Aid and CPR certification
